Banner / Sponsor Request Form
Due to the Privacy Act, we must have permission from the military member or from a relative of the service member to display his/her name on the banner. It can be stated in the body of an email then sent directly to email@example.com
We require the email address of both yourself and of the service member (if requesting banner for yourself, put your email in both places). Your information is confidential and will never be shared or sold.

If sponsoring or donating, use the donate button after submitting. Also, we will need an email granting permission to use the service member’s name on a banner.
